Frustrating and unhelpful medical appointment experience

I urgently required a fit-for-work medical certificate. I arrived at 8am for my appointment and explicitly stated my need for the document. Despite scheduling a 10:15 slot, the doctor didn't see me until 10:50, running 45 minutes behind. The physician then refused to issue the paper, claiming I hadn't been sick for a full week, and directed me to collect a self-assessment form from reception. This makes no sense to me, as a previous doctor had no issue providing the same document when I was ill for only 3 days. The inconsistency is frustrating and illogical.
