Medical notes summary felt unprofessional and unfair

I received medical notes from the surgery that I had requested, but found the handwritten summary of my medical history to be unprofessional and dismissive in tone. The clinician who prepared the notes had never met or spoken with me directly. Particularly concerning was how a medication I had been prescribed but chose not to take—as it was only needed if required—was marked as 'did not take.' This notation referred to a period five years earlier when I was not even registered as a patient at the practice. The way the account was worded suggested I had sought help for various health issues but deliberately refused treatment, which is not an accurate reflection of what actually happened. This misrepresentation fails to acknowledge the genuine effort and commitment patients invest in managing their own health. People's lives extend beyond their GP practice, and inaccurate medical summaries can have serious real-world consequences. In my situation, this affected an important application that I had invested considerable time and effort into pursuing. I would hope that in future, clinicians take the time to engage directly with patients before documenting their medical history, and that they consider how their accounts might impact others' lives. I also hope that should this clinician ever need support or services outside their professional role, they receive compassion and understanding rather than the dismissive approach I experienced. It appeared as though unnecessary effort was made to present my medical history in a negative light, when all that was required was a straightforward provision of medical notes. There are many conscientious healthcare professionals, and unfortunately this does not appear to be one of them.

The practice provided conflicting information regarding vaccine eligibility to a mental health service user. An initial confirmation was given, but this was withdrawn the day before the scheduled appointment. This represents a recurring issue that has occurred more than once.

Updated October 2023: Without question, this is the finest practice I have experienced. All staff members demonstrate helpfulness and genuine care. Their operations run smoothly and their processes function effectively. I can readily access support whenever required, and it is evident that their administrative and appointment management operates to a high standard behind the scenes. I have attended appointments with numerous clinical team members here and have found them all to be excellent. Original 2019: This is an outstanding surgery that makes obtaining healthcare straightforward and comfortable. The reception team are welcoming and very supportive. Getting in touch with the surgery by phone is straightforward and availability for appointments is good. I have attended appointments with clinical staff who made me feel reassured and were keen to assist in any way possible. This is an excellent environment for managing your health. I would gladly recommend to those I know. Thank you.

Extremely pleased with the warm and attentive approach demonstrated by the entire team, who collaborate effectively to prioritise patient wellbeing from initial contact through to final consultation. Quick appointment when calling in today

Our family feels tremendously appreciative of this practice, which we consider to be outstanding and provides prompt, high-quality care to the local community. Having worked within the healthcare sector, I am frequently aware of feedback regarding difficulties in accessing GP appointments elsewhere. However, our own experience has been notably different, and we feel fortunate to have such a capable practice serving our area. When I contacted the surgery today requesting a consultation, I was seen by a clinician within approximately two hours, which we found to be excellent service. We are genuinely impressed by the professionalism and standard of care provided by the team.
