Booked appointment without telling me

I have submitted the online consultation form multiple times now and have also telephoned the surgery on two occasions requesting a callback. During one of these calls, I was informed that an appointment had been arranged for me the previous day, however I was not notified of this. I remained available by my phone at work as I had been led to expect a call. This morning I called again and completed another online consultation form, but I continue to be in a waiting situation without resolution.
