Appointment delays and poor handover notes disappointing

I left my appointment with the nursing staff today feeling frustrated. I had seen another member of the nursing team the previous week and discussed my emotional difficulties. The clinical notes from that earlier visit were inadequate, which made me feel embarrassed and as though I was taking up valuable time, particularly given that I had been struggling with medication adherence and my mood. Combined with being called in 15-20 minutes after my scheduled appointment time, I felt let down by what seems to be a deteriorating standard of care at the practice.
