Appointment letter delay caused months of waiting

I visited the practice in early 2020 with concerns about a health matter and was scheduled for an ultrasound scan. I was informed the appointment would be arranged within approximately three weeks. However, after five weeks had passed without hearing anything, I contacted both the practice and the hospital to follow up. It became apparent that the referral letter had not been sent through. Eventually, I managed to secure a cancellation appointment at the hospital several months later, where I received my scan results. Subsequently, I received a text message indicating that a clinician would contact me at a specified time, but this call did not materialise.
