Compassionate care helps elderly patient get quick support

My elderly father, aged 93, required assistance. The medical practice's phone message was not helpful, so I went to their office and, after some persuasion, met Ryan, the practice manager. Ryan was exceptionally compassionate and, within an hour, Rachael, the Advanced Nurse Practitioner, had visited my father's home to evaluate his condition. In just ten minutes, she contacted the Frailty assessment team at Warwick hospital, and within an hour, my father was at the unit undergoing a comprehensive assessment. The entire process was swift, efficient, and seamless. The reason I'm not giving a perfect rating is that this was our first encounter with the frailty assessment centre, and I believe a 93-year-old living alone should have been identified for assessment earlier to establish an appropriate care plan. Nevertheless, I want to express my gratitude to Ryan and Rachael for their prompt, empathetic, and appropriate intervention.
