Confusing last-minute telephone appointment

I'm utterly frustrated with this medical practice. Despite being a patient here for 7 years, I've only recently started needing appointments in 2024 following a new medical diagnosis last year. After completing a triage form, a doctor noted a telephone appointment in my records. When I checked my NHS app, there were no upcoming appointments, forcing me to call the surgery and wait over 10 minutes just to speak with someone. The receptionist was completely bewildered by the doctor's note, unable to locate any appointment in my records. Later that day, the receptionist called to inform me a remote appointment had been scheduled for 3:30 (by the time I answered, it was already 3:20). The communication breakdown was shocking - I should have been promptly notified when the appointment was initially booked. Had I not independently checked my app and seen the record note, I would have remained completely unaware of the appointment until the 3:30 call. I was also dissatisfied that the appointment was arranged with an external provider rather than my actual surgery. While I understand the rationale from a professional perspective, the medical issue I wished to discuss would have felt far more comfortable addressing with a GP from my registered practice.
