Dangerous delay in urgent medical referral process

The practice used to be top-notch, with the GPs still maintaining their high standards. However, the administrative side is woefully inadequate. I was initially relieved to secure a same-day, in-person GP consultation after reporting potential TIA symptoms. The GP promised an urgent referral to Southmead. When no communication followed, I contacted Southmead directly, only to discover that critical referral information was completely absent, and their previous email to the surgery had gone unanswered. I took it upon myself to alert the surgery and was promised immediate action. Predictably, nothing happened, and I was forced to follow up again today before an admin staff member finally dispatched the necessary documents. Instead of receiving prompt attention for a stroke-related "red flag" condition, I'm now facing a week-long delay. I recognize the immense pressures facing the NHS and its staff shortages, but there's a fundamental issue here: when something is marked as urgent, it genuinely needs to be treated as such - prioritization is not optional!
