Dangerous medical errors nearly cost me my life

I received a prescription for an excessive medication dosage that potentially could have induced a coma. The medication made me extremely sick, and during our final discussion about the incident, I was dismissively told that my condition couldn't have been serious since I hadn't called emergency services. In reality, I was physically incapacitated and actually lost consciousness. On another occasion, while reviewing my medical records online, I discovered a nurse had documented that I was a smoker who declined smoking cessation support—a complete fabrication, as I have never smoked. Additionally, another entry claimed that a nurse had performed an abdominal examination, which never occurred.
