Dangerous prescription leads to medical emergency for elderly patient

My mother was compelled to switch to this medical practice following her hospital stay and rehabilitation in a care facility, which uses this particular practice. After three weeks, she was discharged home. Just days later, she fell ill. A potential urinary tract infection led to an ambulance visit, and the GP from this practice prescribed an antibiotic that she is known to be allergic to. Consequently, she's now struggling to breathe, requires oxygen, suffers from severe chronic diarrhea and extreme vomiting, and is currently in the emergency ward. How could this happen?? Her allergy was clearly documented in her hospital notes... how can a GP who has never even met my mother think it's acceptable to prescribe such a medication? Where has the communication completely broken down??
