Delayed response to hospital discharge recommendations

I rated my experience with the surgery as very poor due to inadequate follow-up care after my hospital discharge and the failure to promptly act on a clearly outlined hospital treatment plan. The discharge documentation and specialist correspondence were detailed in specifying the recommended medication and the reasons for continuing it pending further specialist review. However, I experienced delays and confusion regarding the prescription process, even after the hospital made direct contact with the practice requesting an urgent assessment. The absence of prompt action resulted in a worsening of my health and caused considerable worry for both myself and my family. I felt that the explicit guidance provided by the hospital was not treated with sufficient importance and that there was a gap in communication and coordinated care between hospital and primary care services. In summary, I believed that the practice's response did not appropriately reflect the gravity or time-sensitive nature of the situation, despite the hospital documentation being unambiguous and detailed.
