Five forms later and still no help

I am writing to formally express my profound disappointment with the abysmal service provided by this GP practice. Over several weeks, I have been forced to submit the identical form five times for a straightforward, time-critical letter. Despite explicitly explaining the urgency of my request and persistently following up, I have received absolutely no response, update, or indication of any action. This repeated failure to handle basic administrative tasks is completely unacceptable. The administrative staff's conduct has been consistently unprofessional. They have proven dismissive, unhelpful, and frequently bordering on rudeness. It is evident that there is a total breakdown of communication, coordination, and accountability within the practice. As a patient, I should not be compelled to repeatedly chase the same request or feel unreasonable for expecting minimal service standards. Such negligence creates unnecessary stress and fundamentally erodes patient confidence in the practice's ability to deliver even the most basic healthcare support. Patients have demanding lives and depend on their GP to act efficiently and responsibly—a standard that has been comprehensively missed here. If this situation is not immediately resolved, I will have no alternative but to escalate my concerns to appropriate regulatory authorities. These failings are intolerable, and the practice must urgently address these serious deficiencies. If possible, I would assign zero stars. I demand to speak with the practice manager, as significant changes are imperative, or this practice risks complete collapse.
