Form completion delayed despite repeated follow-ups

I have been waiting for a form to be completed for several weeks following a scheduled phone appointment. The clinician did not call me on the arranged day. After more than a week has passed since the appointment with no contact, I contacted the surgery repeatedly. Each time I was informed that management had been notified, and I was assured that the surgery would update me once they had spoken with the clinician. Despite these assurances, I received no follow-up communication. I was advised to call the surgery the next day if the clinician did not ring in the evening. I am dissatisfied with my experience at this practice and have decided to register with a different GP surgery.
