Frustrated with mental health care follow-up

I've been struggling with this general practice since registering recently. This marks my 4th appointment with the same clinician regarding my ongoing mental health challenges since late October 2024. Each time, I find myself repeating my entire background, despite the clinician being familiar with my history. There's a complete absence of follow-up care or check-ins to monitor my progress or emotional state. The expectation seems to be that I'll manage everything independently. They don't even extend basic communication courtesy like a phone call or text message, even when there's nothing significant to report. I initially chose this GP practice because it was conveniently located and had numerous positive reviews, but my experience has been disappointingly negative. Update 10/06/25- Additionally, I received a hospital report from Watford General Hospital concerning my mental health and suicidal thoughts on 1st June, yet the practice has made no attempt to contact me. I've only been able to initiate communication by calling and submitting an Anima form, and I'm still awaiting a response. With no communication from my designated doctor, I'm seriously considering switching to a different GP practice.
