Frustrating and unprofessional customer service nightmare

I phoned their office to schedule an appointment, and a staff member greeted me by name and birthdate but struggled to locate my records. After a frustrating 10-minute conversation, they informed me that I had apparently moved recently and voluntarily deregistered from their medical practice. Seeking clarity, I visited their location today with documentation to re-register. When I inquired how my GP could have been changed without my consent, the staff member became irritated and dismissively claimed ignorance. Instead of offering an apology or taking responsibility, their attitude suggested that I was somehow at fault for this administrative mix-up.
