Frustrating clinic fails to support patient needs

I am currently attempting to transfer medical providers due to relocating, and I'm encountering significant obstacles in obtaining my essential medications. I've reached out to the clinic multiple times (including a personal visit that required a 30-minute trip) regarding my warfarin prescription, and each interaction has been frustratingly unproductive. They consistently claim they cannot prescribe my medication without seeing my anticoagulant book - a new requirement I was not previously informed about. Despite my repeated explanations that I am in the process of changing clinics, they repeatedly promise to remove the prescription block, only to reinstate it without any communication to me. Moreover, they have inexplicably blocked my inhaler prescription as well, with no apparent justification. I have spent considerable time and effort traveling to pharmacies, only to find my prescriptions are not available, despite the clinic's assurances that they would be processed and sent out.
