Frustrating decline in medical practice quality

The quality of this service has deteriorated due to apparent administrative shortcomings. My medical records have mysteriously lost certain diagnoses and recurring prescriptions, likely from misinterpreting specialist correspondence. Moreover, since the econsult service was discontinued, its replacement appears to be dysfunctional. Recently, I've attempted to submit sick note and GP report requests through the designated administrative channels, only to receive generic, recycled responses that redirect me to the same forms or suggest scheduling an unnecessary appointment. The responses frequently seem disconnected from my original inquiry, demonstrating a lack of attentive and personalized communication.
