Frustrating experience with poor communication and service

I've been a patient at this medical practice my entire life, and the quality of care has steadily declined. Reaching them by telephone is incredibly challenging, and on several occasions, the reception staff have been extremely discourteous. Two months ago, I managed to contact them about my back issue, and they promptly arranged an MRI, which initially seemed promising. However, after that, everything stalled. When I followed up after two weeks, they informed me about the result and mentioned a referral to Ipswich, but nothing progressed. No one pursued the matter until I chased it up a month later. Today, when I called, they claimed I had been referred, yet the NHS has no record of me in their system. There has been absolutely no communication from their end unless I initiate contact. I'm left wondering if any action would have been taken had I not been proactive. I feel completely abandoned and let down - no letters, no phone calls, just total neglect.
