Frustrating healthcare experience with poor communication

My recent blood retest appointments have been utterly frustrating. As someone with a visual impairment, I struggle to read the board indicating when the doctor is ready to see me. I previously communicated this difficulty during my prospect surgery, and it was documented in my notes that staff should assist me. However, in my last two appointments, this accommodation was completely ignored. Additionally, during my most recent blood test appointment, the nurse claimed there was no information in the system about my required blood tests, despite being on medication that necessitates regular monitoring. It's absolutely absurd that since the surgeries merged, all prospect surgery patient records seem to have mysteriously disappeared, leaving me in an incredibly challenging and disorganized situation.
