Frustrating healthcare experience with poor communication

How is this even possible? I received an unexpected phone call about a shingles vaccination, with a proposed date and time that didn't work for me. No attempt was made to reschedule, and I was told the surgery would call me back. Months later, I'm still waiting. I got two text messages requesting a blood test and blood pressure reading. When I tried calling reception, I made no progress and was simply referred to RBH for blood work only! As a patient who rarely uses the surgery's services, I feel completely overlooked and unimportant. What steps can I take to meet your medical requirements?
