Frustrating healthcare experience with poor patient service

I've been a patient at this medical practice for a considerable time, but the quality of service has drastically declined over the past year. Recently, I went to update my surname and request an appointment, only to be instructed to call back later when slots become available. I spent four frustrating hours trying to book an appointment, and when I finally got through, I was offered a telephone consultation in two weeks - not even an in-person meeting! The service is absolutely unacceptable, and I'm seriously considering changing healthcare providers. I'm deeply worried that my ongoing health issues could be something serious, and this inadequate approach to patient care is incredibly distressing.
