Frustrating medical practice with terrible communication

I've been a patient at this medical practice for around three years, and while things initially seemed fine, I've grown increasingly frustrated with their communication and customer service. The phone system is consistently problematic - staff frequently put calls on hold and disconnect, and navigating their menu options is challenging just to reach a live person. When I submit medical requests, whether for minor health issues or treatments, they promise to call back or have me call the next day, but there's always an excuse. The doctor is perpetually "doing paperwork" or needs more time, forcing me to persistently follow up. In one instance, I waited three days just to get a response about a rash cream prescription. Their scheduling is equally inefficient - requesting something on Monday results in being told to come in Thursday or Friday, which means 4-5 days of waiting and chasing them for information. This pattern is frustratingly consistent across all my interactions. The reception staff are unprofessional and dismissive, often chatting among themselves while patients wait. Aside from Sally, the team seems completely disinterested in patient care. What's most distressing is their complete disregard for my mental health. Despite knowing I struggle with severe anxiety and have experienced suicidal thoughts, they remain callous and insensitive. I've attempted to discuss my concerns with management multiple times, yet their rudeness continues. On several occasions, they've been so belittling during phone calls that I've explicitly shared my mental state, only to be met with further dismissal - they've even hung up on me, leaving me in tears.
