Heartbreaking medical nightmare destroying family's trust

Absolutely appalling service! I called mere minutes ago inquiring about my grandfather's pain medication (who is battling cancer), and the staff member refused to assist me because I couldn't provide three forms of identification. I explained I would call back after gathering the required information, only to find their phone lines disconnected! They know exactly who my grandfather is - he's lived in the bungalow next door to the medical practice for over two decades!! The medication should have been processed a week ago!! Total chaos! A doctor visited my grandparents last week, and we were clueless about the purpose until we discovered it was to discuss end-of-life care - and we hadn't even been informed about his cancer diagnosis!! An appointment was scheduled, but the notification letter arrived after the appointment time!! Shockingly, they directly contacted my grandfather, who is also suffering from dementia, and told him about his cancer!! My grandmother and I were blindsided, learning this devastating news from a doctor discussing end-of-life arrangements!! With 14 years of experience in social care, I've never encountered such a heartless, incompetent service in my entire life!! I will be at their surgery tomorrow during my work break to demand the care my grandfather desperately needs!! All I want is for my grandfather to receive his pain relief and vitamin shake!! Please help!
