Heartless letter breaks elderly mum's spirit

My 84-year-old Mother received this correspondence!! REPREHENSIBLE! My 84-year-old mum is so deeply distressed by this letter that she adamantly refuses to contact the doctor at all, declaring she would prefer to endure pain rather than make the call! I attempted to speak with Ferial Kazzaz but was informed that as a Practice Manager, she does not interact directly with patients - instead, I was redirected to the Patient Liaison Officer, to whom I inquired whether the letter had been reviewed prior to distribution - their response was an unequivocal No! My mother has attended 6 appointments since January 2021 - three of which were consultations to discuss follow-up care after her hospitalization following a fall in February - one appointment addressed an allergic reaction to prescribed medication - another was initiated because my mother had to remind the surgery that her routine blood tests were overdue by more than three months. I am so incensed by this letter that I firmly believe the Practice Manager Ferial never even examined the clinical records to understand the nature of these appointments and mechanically dispatched a heartless communication!!
