Heartless receptionist triggers anxiety and tears

My usual experiences with the GP were positive, but one receptionist triggered a panic attack by treating me appallingly when I came to collect blood test forms. My GP had specifically asked me to visit the surgery, and I waited 20 minutes while the receptionist ignored me, engrossed in her computer. Eventually, she harshly demanded to know why I was there, suggesting I shouldn't be present. I explained that it was 10am, the doctor had called me in for forms, but she instantly claimed the forms weren't there without even checking. She then forced me outside, locked the door, and later pushed through a form with a completely different name. When I pointed out it wasn't my name, she insisted it was, as if I wouldn't recognize my own identity. Living with generalized anxiety disorder makes going out challenging, especially during covid, and I'm already on beta blockers alongside anxiety medication. She eventually provided the correct form after verbally attacking me, leaving me in tears. To make matters worse, when I was redirected to another medical center for blood tests, they informed me they'd stopped conducting them six months earlier. I'm not confrontational and followed all guidelines, double-checking covid rules before visiting. This occurred around May 2020, and the experience was so traumatic that I subsequently left the practice, becoming too anxious to return or seek medical help.
