Lifelong patient's trust shattered by medical centre's failures

I've been a patient at Broadwater Medical Centre practically my whole life, having been registered there since I was around 2 years old, and I'm now 24. They are intimately familiar with my comprehensive medical background, encompassing both my physical and mental health challenges and various conditions. While I've generally had minimal problems with the practice (aside from a few specific GPs I actively avoid due to past interactions), the past year has been incredibly challenging. Between medication errors, communication breakdowns (including instances of what I perceive as deliberate misrepresentation), and a particularly distressing incident where a GP triggered an autistic meltdown by verbally berating me throughout an entire appointment, I'm seriously reconsidering my continued association with this medical center. Naturally, I'm hesitant about leaving, as this has been my sole consistent healthcare provider and feels deeply familiar. Nonetheless, I am definitely proceeding with filing a formal complaint with the NHS.
