Missed Details at Follow-Up GP Appointment

I took my wife to see her general practitioner two weeks after her discharge from the medical assessment unit following a minor stroke, with the intention of checking her blood pressure and conducting a blood test. Unfortunately, the doctor had not reviewed the discharge summary, which specifically requested these actions, and instead scheduled blood tests for three weeks in the future. Additionally, the physiotherapist from the stroke team had noted that the GP was expected to refer my wife for Parkinson's tests, but this was not addressed during the appointment. We are now reaching out to the stroke team at the GWH ward that initially discharged her. The GP also claimed that my wife was still under the hospital's stroke team's care, but when we contacted the ward, they confirmed this was incorrect. Once a patient is discharged with a summary, they become the responsibility of their GP, who should follow up on the recommended actions outlined in the discharge documentation.
