Neglected patient struggles with mental health support

In early May this year, I was signed off work due to anxiety and depression. I called weekly to obtain sick certificates for work purposes. As my condition worsened, I reached out to Talking Therapy. This department was absolutely exceptional, and I am now receiving 12 weeks of CBT. Throughout nearly 6 months, this surgery has only contacted me twice, and solely because my therapist and DWP initiated communication. When I finally received a call from a supposed doctor, she bizarrely asked me what I wanted! She was unaware that the contact was requested by DWP while I was experiencing severe suicidal thoughts. As of now, I have received absolutely no meaningful assistance from this surgery in any capacity. If not for the necessity of sick certificates for work, I could have potentially taken my life, and this surgery would have remained completely oblivious and indifferent. I am pursuing formal action as this surgery is now a complete embarrassment and could potentially endanger lives through their incompetence and unprofessional conduct. UPDATE After lodging a complaint, I was granted an appointment with an outstanding mental health professional. When I requested my sick certificate, I was told I must first consult a doctor. During a phone consultation, the doctor stated she would provide a two-week certificate, declaring that "returning to work is the best treatment for anxiety and depression." This so-called professional clearly hadn't reviewed my medical history, even questioning who suggested I should be off work, despite my previous suicidal state! The service is utterly disgraceful, demonstrating a total lack of care, and I intend to pursue this matter to its fullest extent.
