Negligent care delayed my crucial health screening

On December 19th, I visited the practice and was informed I would be referred for an ultrasound regarding testicle lumps. After hearing nothing, I inquired via PATCHS. An initial appointment was cancelled due to the doctor's illness. When I attempted to reschedule, I was told I needed to book an in-person surgery visit. Upon calling to question this, I was assured the doctor was handling the referral. A week passed with no communication, and when I called today, the receptionist revealed the referral was made on May 2nd. I asked why no action was taken during my December appointment, but received no explanation. It appears the doctor simply forgot to process the referral. If I potentially have testicular cancer, a timely referral could have been crucial for early diagnosis. I now want to file an official complaint but lack the manager's direct contact information. At this point, I have no idea how long I'll wait for the ultrasound, and I'm absolutely furious with the entire situation.
