Prescription records don't match what's actually issued

I have serious concerns about my experience at this surgery. There appear to be discrepancies between what is recorded in my patient access records and what is actually prescribed, which has caused significant problems managing my medication throughout the month. I often find myself without adequate supply to cover the full prescribed doses, leaving me with periods of discomfort as I have to ration my medication. When I have brought recent test results from other healthcare providers, these do not seem to be acknowledged or acted upon, and I am not informed about what further testing might be needed or when. This lack of communication makes it difficult to plan my time effectively. My attempts to raise these concerns through email complaints have gone unacknowledged. It was only when I involved another healthcare professional from my treatment team that the situation was finally addressed and I received the medication I should have been prescribed for some time. I find the standard of service and attention to detail deeply disappointing. The lack of care and professionalism is concerning, particularly given how vulnerable patients depend on accurate prescribing and clear communication. I would not recommend this surgery and would change providers if I were not dependent on the regular medication I receive here. This experience has been extremely frustrating and has left me questioning the level of care provided to patients.
