Reception wrote down medicines instead of issuing prescription

I attended the practice recently to collect my prescription for a nutritional deficiency. I was provided with a handwritten note listing medications, which the receptionist indicated had been advised by the clinician and could be taken to any pharmacy. I have concerns about this approach, as it appears the practice may be moving away from issuing formal prescriptions. I am uncertain whether this method is safe or appropriate, particularly if any adverse effects were to occur, as there would be no official documentation to refer to. I question whether this practice complies with proper procedures.
