Receptionists unhelpful, waited weeks for callback

I have been a patient at this practice for over 40 years and today's experience was particularly frustrating. I have been unwell and off work for over a week, so I called the practice as one does to request a sick note. When I finally got through, I asked to speak with a doctor, but the receptionist was unhelpful. I was then transferred to another receptionist who was quite rude. When I asked to speak with my doctor, I was told there would be a two-week wait, or alternatively a one-week wait to see a different doctor. I was eventually told a doctor would call me back, but this call never came. I cannot even recall my doctor's name, it has been so long since I have seen them. The waiting times are extraordinary – two weeks just for a doctor to return a phone call seems unreasonable. More recently, I called again as I needed a work note due to discomfort caused by supplied footwear that is causing me pain. I was informed it could take up to five weeks for a response and three weeks to see my doctor. All I need is a simple work note so I can wear more suitable footwear instead of what has been provided. This is not an unreasonable request, and I am concerned about the impact this is having on my health.
