Registration forms lost again

I submitted my registration and my son's in August, with the staff member stapling the forms together and assuring me that once I was registered, my son would be too. A few weeks later, when attempting to schedule his immunizations, I was informed that a new application was needed as his original form had been lost. I completed and submitted another form on September 16th. For the past two weeks, I've been repeatedly calling the GP to book his appointment, but he remains unregistered. Each time, I'm told they'll search for the form and call back, yet I haven't received a single update. Additionally, no matter when I call, I'm consistently left on hold for 40-50 minutes before anyone answers. Frustrated, I plan to visit the office in person tomorrow to resolve this ongoing issue.
