Renewal form lost after months of phone calls

I submitted a request for a renewal document in April. I called almost weekly to check on its progress. The staff repeatedly told me to call back the following week. This situation continued from April onwards. When I visited in person last week, I was informed that my form had gone missing. I was very surprised and asked the staff how this could have occurred, especially given that they had been asking me to phone back each week. I have now been given a replacement form, and I have requested written confirmation from staff that they have received it. This experience has been quite frustrating.
