Repeated phone cuts and delayed medication after discharge

This practice appears to be struggling to meet its responsibilities. An elderly patient was discharged from hospital with instructions to contact the surgery immediately, as their medications had been substantially altered. It took multiple attempts on that day to reach someone, and the hospital had to be contacted twice on the patient's behalf. When contact was finally made, assurance was given that the updated medication list had been received and would be prioritised for urgent processing to ensure timely delivery. However, a week later, the medications had still not arrived. Upon investigation, the pharmacy confirmed they had not received the prescription, and several calls to the surgery revealed that the in-house pharmacist had not yet authorised it. Subsequent attempts to contact the surgery for clarification were unsuccessful due to repeated call disconnections. The patient currently has medication for today but none for tomorrow. This situation is deeply concerning. The experience raises serious questions about how vulnerable patients without an advocate to support them would manage in similar circumstances, and whether it is reasonable to expect families to spend considerable time following up on tasks that should be completed by the practice.
