Serious medical errors could have harmed my daughter

I'm going to have to reach out to this medical practice. My daughter's hospital discharge summary listed a medication she has never been prescribed as a long-term treatment. Thankfully, to the best of our knowledge, she was not administered this medication during her hospital stay. Her hospital consultant is investigating the matter. They erroneously recorded that she is on two heart medications. She is not and has never been on these drugs. If she had been given these during her hospital stay, they could have potentially caused heart damage. Upon reviewing her GP records, these medications were noted as regular prescriptions, supposedly first prescribed in the year 1900. If medical staff only check prescribed medications upon hospital admission without verifying the dates, she could be inadvertently given these drugs. They have also added notes to referrals following A and E visits, suggesting no further action is required. You should be able to trust that they won't put you at risk like this, without having to act as a detective every time you interact with them.
