Terrible medical care during my mother's final weeks

My family members have registered with this practice, and they've noticed a significant decline in service quality over recent years. Specifically, my 101-year-old mother was seriously ill with the flu, yet no doctor felt compelled to personally examine her condition - we were essentially left to manage on our own. After persistent requests, we finally secured a doctor's visit, who initially assessed her as not critically unwell. However, her health rapidly deteriorated, prompting another call to the surgery where we were informed she was entering 'end of life' stages. This was supposedly meant to trigger additional support for her care. Disappointingly, it took a full week after that communication before any support materialized, and we only received assistance for two days before her passing. Ultimately, I'm deeply skeptical about how this medical practice evaluates its performance - in our experience, their care fell drastically short, and we endured seven weeks of absolute anguish from her flu onset until her death.
