Terrible service and inefficient maternity form process

I strongly advise against this medical practice due to their abysmal and ineffective reception service. Back in February 2023, on July 14.07, I requested a Maternity Exemption Form from reception, which I urgently needed after obtaining a prescription without payment, as NHSBSA demanded this document immediately to prevent penalty charges. Each month, NHSBSA emails reminding me they haven't received my form from the GP. I've completed this form four times at reception, consistently being assured they sent previous forms and suggesting postal service issues might be responsible. Throughout this process, I've contacted NHSBSA for updates and paid prescriptions to avoid charges, ultimately requesting to personally deliver the signed form to NHS Authority via special delivery. Upon collecting the signed form, I was stunned to receive the original document signed by the doctor in February. When I questioned the receptionist about why the form remained unsent for five months, their response was a mere "Apology and Good Luck in the future," which is completely unacceptable.
