Terrible service for vulnerable elderly patient

The medical practice shows minimal concern and responsiveness regarding my elderly father's healthcare needs. His COPD has been neglected for an extended period, and I've been forced to persistently contact them via phone and written communication to prompt any action. The process of obtaining a dementia assessment has been excruciatingly slow, with no formal diagnosis yet established. Despite promises of a nutritionist visit, we continue to wait. A recent telephone COPD consultation was completely ineffective, given my father's homebound status and dementia condition. While staff members can appear pleasant and supportive during conversations, ultimately their commitments are empty. The surgery consistently makes assurances but fails to follow through on any meaningful medical support.
