Very poor communication

I have been with the Hackwood Surgery through all it's various guises for over 50 years and in all that time never really had any cause for complaint. In this time I have had two very good GP's who knew me very well, Dr. D.E.Side and Dr. P.G.R. Wright. I was prescribed a medication by a consultant at BDH some two years ago and was told I would be on the medication for ever. I have annual reviews with this consultant, the last one being on 18/6/2026. I recently put in a repeat prescription request for this medication plus another medication I take daily for hypertension. On collection the prescription from Boots in town I found that the medication prescribed by the consultant was omitted. Boots informed me that the medication had not been requested. On visiting the surgery to fill in another repeat prescription request form I learnt that the medication, prescribed by the consultant, had been deemed 'medication review' so not prescribed. I had not received any communication from the surgery as to their decision. Question 1. Who/why had this medication been deemed 'medication review' despite the review by the consultant in June. Question two: Why/why in CGH had decided to disregard the review by the consultant (I have a copy of the letter sent by the consultant to CGH on 18/6/2026. Question 3: Why had CGH decided to not inform me of their decision. I am left without medication until a phone, YES a phone review at the end of July!! There appears to be a lack of 'joined up' thinking by these 'health professionals' at this group of surgeries!!
